The number of residents living in nursing homes nationwide has declined at a steady pace over the last 10 years. More and more people each year are seeking aging in place support so they can remain at home.
“I’m interested in helping people stay as healthy in their homes as possible,” said Leta Chen, a care manager for JSSA’s Aging in Place program. “If I can promote a self-management approach to my clients, they can be empowered to remain in their homes.”
The realities of aging – and the potential for physical and mental decline – can make continuing to live at home challenging for some as they get older. But aging in place support is available.
At JSSA, Leta serves a growing caseload of aging adults living in Silver Spring, Md. All her clients live on a limited income and hope to stay in their apartments as they age. As part of our partnership with Homecrest House, she helps find and coordinate affordable services and support for them to live a safe and healthy lifestyle at home.
Aging in place support services help prevent falls and visits to the hospital, or in more serious circumstances, having to move to a facility. JSSA’s caregivers are committed to helping people be empowered to live where they choose.
“With enough support, older adults can successfully age in place,” Leta said. “Sometimes people just don’t know what’s available.”
Aging in Place Support
Leta said that as a case manager, she acts as her clients’ biggest advocate. She finds ways to make “every penny count.” Between rent, food costs, medical care, and emergency planning, many of her clients need her to be creative in finding affordable and accessible support services.
Services include transportation assistance, Homecare, and meal delivery. Additionally, JSSA provides therapy, counseling services, and opportunities to socialize.
